29 CFR  1926.251(a)(6):Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service:  On or about October 13, 2020, at a worksite located at 5995 North Ridge Road, Madison, OH 44057, the employer did not inspect synthetic slings prior to use and remove from service any synthetic slings with no working load limit labels or permanent markings.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about October 13, 2020, at a worksite located at 5995 North Ridge Road, Madison, OH 44057, during commercial carpentry activities the employer did not ensure employees working at the window opening were protected from a fall of approximately 18 feet by utilizing a fall protection system.
