On or about October 19, 2021, during the canopy installation of a CITY BBQ commercial building located at 6372 Som Center Rd, Solon, OH 44139; the employer did not ensure employees working on the from were protected by from possible danger of head injury from impact or flying object by wearing protective helmets while working directly underneath truss setting operations from working surfaces approximately 9.4 feet.

29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 19, 2021, during the canopy installation of a CITY BBQ commercial building located at 6372 Som Center Rd, Solon, OH 44139; The employer did not ensure employees working on the roof were protected from a fall of approximately 9.4 feet by utilizing a personal fall arrest system or any other form of fall protection system. Integrity Construction by Ron Burkholder was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1427213, citation number 01, item number 002A and was affirmed as a final order on 01/21/2021, with respect to a workplace located at 5995 North Ridge Rd, Madison, OH 44057.
