29 CFR  1926.102(a)(1):29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At residential worksite located E26th St and W Paseo Blvd KC MO -- Employees engaged in framing and roofing activities, using a portable DeWalt skill saw and hammer, were exposed to struck-by hazards in that eye protection was not utilized. This exposed employees to struck by hazards. This was documented on or about 10/07/2020.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At worksite located at E 26th St and W Paseo Blvd, KC MO -- employees were applying plywood sheeting and framing of a new residential house. The employees were excess of 26 feet above ground level without any fall protection system. This exposed employees to fall hazards. This was documented on or about 10/07/2020.
