29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  1417 Diamond Station Road Ephrata PA - The employer did not provide training to employee(s) which would have allowed employees to recognize the hazards of falling and the equipment and procedures to use to minimize those hazards,  on or about October 14, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a) 417 Diamond Station Road Ephrata PA - The employer did not have a written certification that  employees were trained to recognize the hazards of falling, the equipment necessary and procedures to use to minimize those hazards, on or about October 14, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)   1417 Diamond Station Road Ephrata PA - Employees were performing roofing activity on a steep residential roof, 8 in 12 pitch without the benefit of a complete fall protection system,  exposing employees to a fall of approximately 24 to 30 feet high at the back of the house and approximately 12 feet high at the front of the house to the ground below, on or about October 14, 2020.    Bottom Line Construction LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1279792, citation number 1, item number 1 and was affirmed as a final order on March 6, 2018, with respect to a workplace located at 1202 - 1207 Chatham Drive Morgantown, PA 19543.  No abatement certification or documentation required.
