29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Water main construction located at the corner of Powder Springs Dallas Road & Angham Road, Powder Springs, GA, trench: On or about 10/20/2020, the employer exposed an employee to struck-by hazards while working in side a trench measuring 6-feet, 4-inches deep, in that head protection was not donned prior to entering the trench as tools, ladders, and equipment were located over head. The employee also worked in an excavation which was not provided with a cave-in protection system which created an additional struck-by hazard.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Water main construction located at the corner of Powder Springs Dallas Road & Angham Road, Powder Springs, GA, trench: On or about 10/20/2020, the employer exposed an employee to cave-in/entrapment hazards while working inside a trench measuring 6-feet, 4-inches deep, in that a safe means of egress was not provided within 25-ft. of the location of the employee installing 8-inch ductile pipe inside the excavation. D & H Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number #1318174, citation number 1, item number 3 and was affirmed as a final order on 07/24/2018, with respect to a workplace located at Tuggle Road NE & West Rock Quarry Road NE, Buford, GA 30519.
