29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  On or about October 19, 2020, in the fabrication shop, the employer did not have written specific energy control procedures that precisely outline the scope, rules and steps for the control of hazardous energy for the HAAS CNC VF-3, among other equipment with potential to store energy, exposing employees to caught-in and amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  On or about October 19, 2020, at the metal fabrication area, the Scotchman band saw was being used without a protective guard at the blade, exposing employees to amputation hazards.

29 CFR  1904.29(a):A log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses, (OSHA Form 300A) and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment:  a)  On or about October 19, 2020, throughout the fabrication plant, the employer did not use the OSHA-300 form, OSHA 301 form or equivalent forms to document recordable injuries and illnesses for current calendar year 2020.  b)  On or about October 19, 2020, throughout the fabrication plant, a copy of the OSHA-300, or equivalent form for all work related injuries and illnesses for years 2017, 2018 and 2019 were not kept for review as required by the standard.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  a.  On or about October 19, 2020, at the south wall of the fabrication building, the 480-V electric panel had circuit breakers being used that were not marked to indicate their purpose, exposing employees to electrical hazards.
