29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about October 21, 2020, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about October 21, 2020, at the above addressed jobsite, employees performing residential carpentry operations, twenty-three (23) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.  Ryehill Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1396059, citation number 1 item number 1 and was affirmed as a final order on October 8, 2019 with respect to a workplace located at 5801 S. Newcastle Ave. Chicago, IL 60638.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about October 21, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.  Ryehill Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1396059, citation number 2 item number 1 and was affirmed as a final order on October 8, 2019 with respect to a workplace located at 5801 S. Newcastle Ave. Chicago, IL 60638.
