The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a) Jos� Luis Barcenas Flores, at a jobsite located at Lot 13489, Newark, Ohio 43055: On and/or preceding October 21, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Framers were using an extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 123 volt electrical contact hazard.

Jos� Luis Barcenas Flores was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1474231, citation number 1, item number 1 and was affirmed as final order on June 16, 2020, with respect to a workplace located at 1743 Silver Lawn Drive, Lot#114, Grove City, OH, 43123.

Framers were using pneumatic nail guns and circular saws in the framing process. The employees were not utilizing eye protection, which exposed them to struck-by hazards to the eyes. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a) Jos� Luis Barcenas Flores, at a jobsite located at Lot 13489, Newark, Ohio 43055: On and/or preceding October 21, 2020, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. Employees was framing and sheathing the second story roof without any means of fall protection, thereby exposed to fall hazards of up to 20 feet.  b) Jos� Luis Barcenas Flores, at a jobsite located at Lot 13489, Newark, Ohio 43055: On and/or preceding October 21, 2020, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. Employees was framing the second story where the window openings were unguarded, thereby exposed to fall hazards of up to 15 feet.  Jos� Luis Barcenas Flores was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1474231, citation number 2, item number 1 and was affirmed as final order on June 16, 2020, with respect to a workplace located at 1743 Silver Lawn Drive, Lot#114, Grove City, OH, 43123  Jose Flores was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1205371, citation number 1, item number 1 and was affirmed as final order on February 22, 2020, with respect to a workplace located at 5762 Somersworth Loop, Lewis Center, Ohio.
