29 CFR  1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures in any workplace where respirators were necessary to protect the health of the employee or whenever respirators were required by the employer:  Employees performing routine operations/tasks activities in the Powder Coat Spray Booth, Wash Booth, and Galvanized Welding Shop were exposed to respiratory hazards such as but not limited to the dust particulates, fumes/mist and vapors in that a written respiratory protection program was not provided where respiratory protection was required to protect the health of employees.  29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.

29 CFR 1910.213(h)(4): Radial saw(s) were not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator:  Employees engaged in assembly activities at the machine shop area were exposed to amputation hazards in that the 10-inch Sears Craftsman Radial Arm Saw would not return to its back stops, or its starting position when released.
