29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 25 Southgate St. Worcester, MA: On January 19, 2021 and at times prior, employees were exposed to potentially hazardous energy when they did not utilize the energy control procedure during morning pre-production set up tasks on Pie Line 1. On October 21, 2020, an employee received finger amputation injuries while oiling a chain driven section of Pie Line 1. Table Talk Pies was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1267712, citation number 2, item number 1 and was affirmed as a final order on March 30, 2018, with respect to a workplace located at 120 Washington St, Worcester, MA.

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed: 25 Southgate St, Worcester, MA: On October 23, 2020 and for many months prior, the employer had not performed a comprehensive periodic inspection of the energy control procedures. The employer only performed energy control procedural reviews and had authorized employees quickly demonstrate procedures on machines. The employer did not ensure each procedure was inspected for inaccuracies during an actual energy control task. Additionally, the employer did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure they were authorized to use. Table Talk Pies was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1267712, citation number 1, item number 1a and was affirmed as a final order on March 30, 2018, with respect to a workplace located at 120 Washington St, Worcester, MA.
