29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees, in that employees were exposed to a crush-by hazard: On or about October 21, 2020, inside the manufacturing department in the machine shop room, a foot pedal was installed on a Lemas TR 100/4 Initial Pinch Roll, without obtaining authorization from the manufacturer of the equipment and allowing employees to place both hands at the point of operation, thereby exposing employees to a crushing hazard. Recommended Abatement Action: Among other methods, a feasible and acceptable abatement methods to correct this hazard is to review and revise the existing safety and health program of the company, to ensure machinery and equipment are only modified and altered with the written authorization from its manufacturer; and instruct personnel on the revisions of the safety and health program .
