29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a)  J-Jireh Contractors, Inc, at 3558 Helena Street, Aurora, CO:  On 10/23/2020 the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to excavations to control or eliminate any hazards or other exposure to illness or injury.  Two employees were directed to enter an approximate 9.2 foot deep, four foot wide, vertically walled excavation to connect sewer pipes.  The employees at the worksite had not received effective training regarding excavation hazards or the use or limitations of protective systems.  This condition exposed the two employees to a cave-in hazard.  (b)  J-Jireh Contractors, Inc, at 3558 Helena Street, Aurora, CO:  On 10/23/2020 the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to work zones to control or eliminate any hazards or other exposure to illness or injury.  A work zone was established on Helena Street during excavation and installation of sewer piping.  A flagger was located at the north end and a flagger was located at the south end of the work zone.   The employer did not ensure that all flaggers were trained and qualified to perform flagger duties.  This condition exposed employees to a struck-by hazard.
