29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: J-JIREH CONTRACTORS, INC at 9140 Huron Street, Thornton, CO 80260: On July 30, 2021 the exposing employer did not ensure excavated materials were at least 2 feet from the edge, in that a spoils pile situated on the east side of the excavation was approximately 6 inches from the edge of the excavation. Employees were required to work in the excavation measuring approximately 9 feet deep in the area beneath the spoils pile. This condition exposed the employee(s) to struck-by hazards.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres and or other hazardous conditions: J-JIREH CONTRACTORS, INC at 9140 Huron Street, Thornton, CO 80260: On July 30, 2021 the creating and exposing employer did not ensure that adequate and effective daily inspections of an excavation were conducted, in that an employee worked inside the trench at a location that measured an estimated 9 feet deep without an adequate protective system in place. A spoils pile situated on the east side of the excavation was approximately 6 inches from the edge of the excavation. This condition exposed the employee(s) and subcontractors to crushing and cave-in hazards.

This condition exposed employee(s) to crushing hazards by excavation wall cave-in.
