Site - On or about October 22, 2020, employees engaged in roofing activities were exposed to falls of over 9 feet where fall protection was not provided or utilized to prevent the hazard.  Jos� M. Acero Chuma d/b/a JMC Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1457975, citation number 1, item number 3, and was affirmed as a final order on 02/18/2020, with respect to a workplace located at 318 Lexington St, Woburn, MA 01801.

Site - On or about October 22, 2020, the employer's competent person did not train each employee in the nature of fall hazards in the work area, the correct procedures for erecting, maintaining, disassembling, & inspecting the fall protection systems to be used and the installation, use & operation of personal fall arrest systems, including anchorages.
