29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   Facility: Engine Bay: Employees had placed a large stand-up fan and multiple large and small cardboard boxes directly in front of an electrical panel located on the wall next to the upstairs storage area.  Employees were exposed to electrical hazards from not being able to access the equipment for maintenance or emergency purposes.

29 CFR  1910.305(d)(2): Panelboards were not mounted in cabinets, cutout boxes, or enclosures designed for the purpose and/or were not dead front:   The electrical panel located on the wall next to the storage area was missing the panel cover. Employees accessed the engine bay daily to access stored supplies and equipment. Employees were exposed to electrical hazards.

29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Facility: Chainsaw Repair Room: Employees who conducted chainsaw repair tasks stored butane cylinders inside a flammable storage locker with liquid fuels, oil, gasoline, or waste. Employees were exposed to fire hazards.

29 CFR  1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:   Facility: Chain Saw Repair Room: Employees used liquid fuels to repair and operate chain saws and stored the Category I liquid fuels in a flammable storage locker with natural ventilation that exhausted into an interior, locked, non-ventilated room containing multiple types of sparking equipment and butane cylinders. Employees were exposed to fire hazards.

Employees were exposed to dermal, eye, and lung irritations.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Facility Buildings: Employees used concentrated bleach, calcium lime rust liquid to clean equipment and the interior sections of the facility. The employees used insecticide on the grounds adjacent to the building where bunk rooms, engine bay, and administrative operations occurred. The employer had not provided an operable eye wash station for use. Employees were exposed to eye irritations.
