29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were allowed to work on energized circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section. Cottonwood Visitor Center and Maintenance Grounds: While working with the solar panel battery bank and the Denyo MQ Power Whisperwatt 25kW diesel powered AC generator, visitor center employees were not capable of working safely on energized circuits and were not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools. Employees were exposed to shock and arc flash hazards.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph: Cottonwood Visitor Center and Maintenance Grounds: While working and troubleshooting operations with the solar panel battery bank, visitor center and maintenance employees worked around electric equipment operating over 50 volts that was not guarded from accidental contact. Employees were exposed to shock and arc flash hazards.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: Visitor Center and Administrative Offices: Employees used Clorox Bleach concentrate, Purell sanitizer, Advanced Caliber sanitizer, and Sensient sanitizer during cleaning process such as cleaning cashier counter, plexiglass panels, educational public displays, shared administrative desks, handrails, and mopping the retail store floor multiple times during the daily work shift hours. The employer had not obtained and retained safety data sheets for the chemicals used in the retail store and administrative spaced. The employees were exposed to eye and dermal irritation hazards.
