29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: a) Fabrication Department - On or about October 26, 2020, employees were performing burning and welding. The employees were provided fire extinguishers to use during incipient stage fire fighting, and the employer did not provide an educational program as required. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement. ABATEMENT NOTE: By this date the employer must either correct the alleged violation or implement a Fire Safety Policy, as outlined in 29 CFR 1910.38(a) and (b) which includes the evacuation requirement of 29 CFR 1910.157(b).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Fabrication Department - On or about October 26, 2020, employees burning and welding on stainless steel, were exposed to hazardous chemicals, such as but not limited to, methylene chloride and hexavalent chromium, and the employer did not provide effective information and training on the hazards of such chemicals as required. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement. ABATEMENT NOTE: Employees shall be informed of the following: 1. The requirements of this section; 2. Any operations where hazardous chemicals are present; 3. The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals and Safety Data Sheets. Employee training shall include at least: 1. Methods and observations that may be used to detect the presence or release of hazardous chemical in the work area. 2. The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area. 3. The measures employees can take to protect themselves such as specific procedures and personal, protective equipment to be used. 4. The details of the employer's Hazard Communication Program including an explanation of the labels received on shipped containers and the workplace labeling system used by the employer; Safety Data Sheets, including the order of information and how employees can obtain and use the appropriate hazard information.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a) Fabrication Department - On or about February 17, 2021, employees were exposed to fire and electrical hazards from various pieces of electric powered shop equipment that were plugged into a relocatable power tap (power strip). Strahman Valves, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1402176, citation number 2, item number 2a, and was affirmed as a final order on August 19, 2019, with respect to a workplace located at 2801 Baglyos Circle, Bethlehem, PA. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records. For Reference: According to UL Whitebook, relocatable power taps rated 250 V ac or less, 20 A or less, intended for indoor use as relocatable multiple outlet extensions of a branch circuit to supply laboratory equipment, home workshops, home movie lighting controls, musical instrumentation, and to provide outlet receptacles for computers, audio and video equipment. In addition, relocatable power taps are not intended to be secured to building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring.
