29 CFR  1910.212(a)(3)(iii): Special handtools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  JOB SITE: 1500 W. Malone Street, Sikeston, MO 63801, on or about 10/29/2020, employees were permitted to place their hands in the danger zone of the Take-up machines while installing the cable wires onto a reel.  Employees were not provided with special hand tools for placing and removing material (cable wire) for the following equipment:  1) The MGS Group, Model: 72 inch MA Dual Semi-Automatic Take-up, S/N: JOB4104 2) SKALTECK, Model: Take-up U22T, S/N: US 2574-2714-30

29 CFR  1910.212(a)(3)(ii): The point(s) of operation of machinery exposing employees to injury did not have guards that were in conformity with appropriate standards or designed and constructed to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  JOB SITE: 1500 W. Malone Avenue, Sikeston, MO 63801, on or about 10/29/2020, employees were permitted to operate take-up machines within inches of the material (wire cable) being rolled onto a reel without any forms of guards to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).  As a result of the process, an employee sustained an amputation injury.  The workers were exposed to a pinch-point hazard.    Equipment:  1) The MGS Group, Model: 72 inch MA Dual Semi-Automatic Take-Up, S/N: JOB4104 2) Skaltek, Model: Take-up U22T, S/N: US2574-2714-30  The ALAN WIRE COMPANY was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.212(a)(3)(ii), which was contained in OSHA inspection number 1109033, citation number 1, item number 1, issued on 1/27/2016, and was affirmed as a final order on 2/12/2016, with respect to a workplace located at 1500 W. Malone Street, Sikeston, MO 63801.
