Tunnel under aggregates silos: On or about November 3, 2020, and at times prior, the employer exposed employees to the hazards presented by ingoing nip points and rotating parts of a conveyor inside the tunnel, in that employees were not trained on the appropriate safe application, usage and removal of energy control devices when employees enter the tunnel to clean the floor and area.
