On or about November 3, 2020, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a. On or about November 3, 2020, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing  employees to the hazards associated with falls.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a.  On or about November 3, 2020, at the above addressed jobsite, an aluminum extension ladder with a broken ladder rung was used to access and egress the roof and was not removed from service, thereby exposing an employee to the hazards associated with falls.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about November 3, 2020, at the above addressed jobsite, employees performing residential roofing operations on a roof fourteen (14) feet four (4) inches above ground were not protected from falls, thereby exposing employees to fall hazard.  b.  On or about November 3, 2020, at the above addressed jobsite, employees performing residential roofing operations on a roof ten (10) feet nine (9) inches above ground were not protected from falls, thereby exposing employees to fall hazard.  Artex Roofing Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1405069, citation number 1 item number 1 and was affirmed as a final order on September 16, 2019, with respect to a workplace located at 4113 147th Street, Midlothian, IL 60445.  Artex Roofing Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1219964, citation number 2 item number 1 and was affirmed as a final order on July 19, 2017, with respect to a workplace located at 27822 135th Street, Plainfield, IL 60585.

On or about November 3, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
