29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about May 16, 2023, employees engaged in residential roofing work were not protected from falling through skylights, exposing employees to a fall hazards. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about May 16, 2023, employees engaged in residential roofing were not provided training to recognize and minimize fall hazards, exposing employees to a fall of 10.6 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system : On or about May 16, 2023, employees engaged in residential roofing work were not protected from falls, exposing employees to a fall of 10.6 feet to the ground below. Artex Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1610899, citation number 1, item number 1 and was affirmed as a final order on December 20, 2022, with respect to a workplace located at 565 - 569 Parkview Lane Lockport, IL 60441. Artex Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1500963, citation number 2, item number 1 and was affirmed as a final order on February 19, 2021, with respect to a workplace located at 1128 Patriot Dr. New Lenox, IL 60451. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
