29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level. Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  In the Subgated area, the Portacool 46 inches diameter fans had an approximately two inches opening on the blade guard, exposing employees to an amputation and laceration hazards.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  In the Welding Shop, the Jet 10 inch bench grinder work rest was adjusted to approximately 1/2 inch from the wheel, increasing the potential for a part to become lodged against the wheel during grinding, exposing employees to a struck by and caught in hazard.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  In the gated and subgated areas, employee engaged in manufacturing and maintenance activities were exposed to chemical hazard which including eye irritation, skin irritation, and vision loss in that the employer did not maintain label, tag, or mark the container with the product identifier in the following instances:  a) Unlabeled cleaner in line 2 of Subgated area  b) Unlabeled chemical in AGV Maintenance area  C) Two containers of unlabeled chemicals in the Welding Shop  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
