29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  In the Machine Shop at the HURCO VMX 64 automated CNC machine, employee(s) were exposed to laceration and amputation hazards in that an Authorized employee engaged in maintenance work inside the CNC machine was not provided with any machine specific lockout tagout procedures for the CNC machine as the employer did not develop any. This resulted in an employee amputation and hospitalization. The automated CNC machine is as follows:  1. Make: HURCO Manufacturing Co.; Model: VMX64/50T; Serial #: M653-02002110A8A; Color: Blue & Tan.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  In the Machine Shop at the HURCO VMX 64 automated CNC machine, employee(s) engaged in maintenance work were exposed to laceration and amputation hazards in that the employer did not conduct yearly periodic inspections. This resulted in an employee amputation and hospitalization when an employee entered an automated CNC machine while it was in automatic mode which caused a tool change that amputated the employee(s) left ring finger.  The automated CNC machine is as follows:  1. Make: HURCO Manufacturing Co.; Model: VMX64/50T; Serial #: M653-02002110A8A; Color: Blue & Tan.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  In the Machine Shop at the HURCO VMX 64 automated CNC machine, employee(s) engaged in maintenance work inside an automated CNC machine were exposed to laceration and amputation hazards in that an Authorized employee leaned inside the CNC machine while it was running in automatic mode and while running an automated program to blow off excess debris from the material being machined without applying a lockout or tagout device to the machine's energy isolating device. This resulted in an employee amputation and hospitalization. The automated CNC machines is as follows:  1. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  In the Machine Shop at the HURCO VMX 64 automated CNC machine and the HURCO BMC 50, the employer failed to protect employee(s) engaged in automated CNC operations that were exposed to laceration and amputation hazards in that the doors and/or enclosures for the automated CNC machines were removed by the employer and the safety interlock was disabled by the employer. This resulted in an employee amputation and hospitalization. The automated CNC machines are as follows:  1.
