(a) Golf Course Maintenance Building - On or about November 5, 2020 and at times prior thereto, employees were exposed to overloading and failure of the AC Delco rolling floor jack in that the rated load was not permanently marked in a prominent location on the jack. In the event of failure of the jack, employees are likely to have golf course grounds keeping equipment such as lawn mowers fall on top of them.

(a) Blue Bills Kitchen - On or about November 5, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation in that three microwaves (SHARP - commercial microwave oven Model No. R-21LCF/Serial No. 159384) were plugged into one Relocatable Power Tap (RPT) Woods No. 5709. UL listed 34C4. (b) Back Area Event Center - On or about November 5, 2020 and at times prior thereto, employees were exposed to fire and smoke inhalation in that four (4) RPT's were connected in sequence (Stanley & Bilkin RPT's).

29 CFR 1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was not provided within the work area for immediate emergency use. (a) Backup Generator Room - On or about November 5, 2020 and at times prior thereto, employees using fertilizer and other grounds keeping chemicals were exposed to eye injuries in that no suitable eye washing station was in the immediate vicinity for emergency use.
