29 CFR  1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical.  (a) 5647 Highway 126, Florence, Oregon 97439, Laundry operations area, on or about August 9, 2023 and at times prior thereto, employees were exposed to serious injuries, disability, and illness due to SDS for hazardous chemical used in the process of laundry operations was not available to employees which would result in skin, eye, and ingestion injuries.

(a) 5647 Highway 126, Florence, Oregon 97439, Human Resources area, on or about August 9, 2023 and at times prior thereto, Employees were exposed to trip and fall hazards due to emergency exit lighting not working properly which would result in body extremity injuries.

29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a) 5647 Highway 126, Florence, Oregon 97439, Laundry operations, main kitchen, and warehouse areas, on or about August 9, 2023 and at times prior thereto, employees were exposed to trip/fall  and fire hazard due to there was not sufficient access and working space provided and maintained about several electrical panels, which would result in serious injuries to body extremities.
