Wichita, KS, the employer did not ensure that appropriate permission from the manufacturer was obtained by the user prior to using an attachment on the JLG Telehandler, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly. This condition exposes employees to struck-by and crushing hazards.
