29 CFR 1910.1096(b)(1):No employ shall possess, use, or transfer sources of ionizing radiation in such a manner as to cause any individual in a restricted area to receive in any period of one calendar quarter from sources in the employer's possession or control a dose in excess of the limits specified in Table G-18. Rems per calendar quarter/ 1.25 rem to the Whole body: Head and trunk; active blood-forming organs; lens of eyes; or gonads. Interventional Radiology Department: An employee was exposed to health hazards related to ionizing radiation. Since the forth quarter of 2017, there have been nine incidents where an employee was exposed to ionizing radiation above 1.25 rems.

29 CFR 1910.1096(b)(2):An employer may permit an individual in a restricted area to receive doses to the whole body greater than those permitted under subparagraph (1) of this paragraph. Interventional Radiology Department, An employee was exposed to health hazards related to receiving a dose of 5030 mrems to the whole body in the third calendar quarter of 2020. It is not possible to determine the allowable dose limit below 195,000 mrems because lifetime ionizing radiation data is not available.

29 CFR 1910.1096(m)(1):In addition to any notification required by paragraph (1) of this section the employer did not make a report in writing within 30 days to the Assistant Secretary of Labor or his duly authorized representative, of each exposure of an individual to radiation or concentrations of radioactive material in excess of any applicable limit in this section. Interventional Radiology Department: An employee was exposed to ionizing radiation above 1.25rem at least nine times during last thirteen quarters. A report in writing was not made within 30 days to the Assistant Secretary of Labor or his duly authorized representative of each exposure of an individual to ionizing radiation. Each report required under this paragraph should have described the extent of exposure of the employee to radiation, the cause of the exposure, levels of concentrations; and corrective steps taken or planned to assure against a recurrence.
