29 CFR 1910.165(e): The employer did not assure that manually operated actuation devices for use in conjunction with employee alarms are unobstructed, conspicuous and readily accessible. In the hallway near exit door to dock, the alarm activation station was blocked by pallets, cart, and traffic cone, and was inaccessible to employees.
