29 CFR 1910.305(a)(2)(ix):Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lamp holder with a guard: (a) On or about November 9, 2020 and at times prior to that date, for the employees and the employees of a contractor under the control of the DeCA Nellis Commissary were exposed to stuck-by and/or electrical hazards where light fixtures were positioned directly over or in close proximity to product pallets moved by forklifts in the warehouse located at 4200 Mountain Home Street, Nellis AFB, Nevada.
