29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a)	Defense Commissary Agency, Joint Expeditionary Base Little Creek: On or about 02/14/2023, the unused openings on the bottom and top of breaker panel PPC in the mechanical room were not provided with a cover.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a)	Defense Commissary Agency, Joint Expeditionary Base Little Creek: On or about 02/14/2023, the unused openings on the bottom and top of breaker panel PPC in the mechanical room were not provided with a cover thus exposing employees to a possible burn resulting from a potential arc flash.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1904.32(b)(6): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) for the previous year was not posted between February 1 and April 30:  a)	Defense Commissary Agency, Joint Expeditionary Base Little Creek: On or about 02/14/2023, the OSHA 300a for 2022 was not posted.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard  29 CFR 1904.32(b)(6) , which was contained in OSHA inspection number 1519949, Notice  number 2, Item number 1 and was affirmed as a final order on 07/14/2021, with respect to a workplace located at Defense Commissary Agency Norfolk 1588 Mall Drive Norfolk, VA 23511.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a)	Defense Commissary Agency, Joint Expeditionary Base Little Creek: On or about 02/14/2023, an employee in the Receiving Department required to operate a Yale Walkie Pallet Trucks Model MPW050E, was not provide with a performance evaluation at least once every three years.   No abatement certification or documentation required.
