29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  On 11/6/2020, employees of Rogelio Salgado were re-roofing a commercial building at 1032-1048 Rondale Court, Dardenne Prairie, Missouri.  The employer did not have competent person(s) conduct frequent and regular inspections of the jobsite.  Employees on the site were exposed to hazards including but not limited to fall hazards and struck-by hazards.

29 CFR 1926.501(b)(10):  "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  On 11/6/2020, employees of Rogelio Salgado were re-roofing a commercial building at 1032-1048 Rondale Court, Dardenne Prairie, Missouri.  The employees were working on a 4:12 pitch roof, which was approximately thirteen feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  On 11/6/2020, employees of Rogelio Salgado were re-roofing a commercial building at 1032-1048 Rondale Court, Dardenne Prairie, Missouri.   The workers were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
