On or about November 18, 2020, employees were exposed to fall hazards while using a portable telescoping ladder to access a roof.  The ladder was not extended at least three (3) feet above the upper landing surface and the employer had not provided training  to the employees to enable them to recognize the hazards and the steps necessary to eliminate or minimize the hazards.
