The 'Fit Check' sections of fit test records were blank for the check boxes marked 'Pass', 'Fail' and 'Not done'. The employer required employee use of N95 filtering facepiece respirators to protect against SARS-CoV-2 while providing care to confirmed COVID-19 positive residents.
