29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l), containing information including the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a) Visual Pak Company located at 1909 S. Waukegan Rd., Waukegan, IL - On or about November 2, 2020, the employer did not maintain certification records that contain the date of training and evaluation or the identity of the person(s) performing the training and evaluation for employees that operate powered industrial vehicles, including the double and triple pallet riders. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
