29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6): 1. Turn off and shut down power to the machinery, 2. Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 3. Affix lockout/tagout devices to each energy isolating device, 4. Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work: a) Visual Pak Company, American Blending and Filling, located at 3505 Birchwood Drive, Waukegan, IL - On or about October 10, 2020, employees were exposed to machine hazards associated with moving parts while performing maintenance tasks on the in feeder, such as cleaning spilled product and removing jams. The employer did not implement energy control application steps as the machine was not shut down or deenergized to perform setup or adjustments [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. No abatement certification or documentation is required for this item.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Visual Pak Company, ABF, located at 3505 Birchwood Drive, Waukegan, IL - On or about November 10, 2020, the employer did not ensure that an evaluation of the operator's knowledge and skill to operate the power industrial vehicles safely was performed every three years for each type of vehicle operated, including the sit down forklift and standup forklifts. No abatement certification or documentation is required for this item.
