29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a) Rene Garcia at 2205 East Philadelphia Street, Rapid City, SD 57701: On or about and at times prior to November 30, 2020 the employer did not ensure that frequent and regular inspections of the job sites had been accomplished.  Employees on site were exposed to various hazards to include, falls, ladder, and training.

This condition exposed employees to a fall hazard of over six feet.

This condition exposed employees to a fall hazard of 10 feet.
