On or about December 02, 2020, the employer failed to conduct periodic inspections annually of their energy control procedures to ensure that the procedures are being followed.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a.) On or about December 2, 2020 the 3 Motionmaster CNC machines along with 1 CIS and 1 Quintax CNC machine did not utilize a guarding method that would prevent employees from getting caught in the rotating tooling and/or stuck by the moving table. b) On or about December 2, 2020 the 3 Brown R223 and 2 AAA Rotary forming machines did not utilize a guarding method that would prevent employees from getting struck by the rotating carousel on the machine nor did they contain a guarding method that would prevent employees from getting their hands or arms caught-in the thermal heating press area. c) On or about December 2, 2020 the 7 Hydraulic Press machines did not utilize a guarding method that would prevent employees from crush and amputation hazards at the point of operation.
