29 CFR 1928.57(a)(6)(i):The employer did not Keep all guards in place when the machine is in operation; a) On or about November 25, 2020, employees were exposed to caught-between hazards while working in and around a conveyor system at the heat barn of plant #4 upper conveyors. The employer did not ensure that conveyor equipment is guarded to protect against caught between hazards. The lack of machine guarding to protect against caught between hazards can result in injuries including but not limited to fractures and amputations.

29 CFR 1928.57(a)(6)(v): The employer did not ensure the lock out of electrical power before performing maintenance or service on farmstead equipment. a) On or about November 25, 2020, employees were exposed to caught-in hazards while performing cleaning and maintenance work in and around a conveyor system at the heat barn of plant #4 upper conveyors. The employer failed to ensure that conveyor equipment was locked out prior to performing servicing and maintenance activities. An employee sustained serious bodily injuries when pulled into the conveyor system.
