29 CFR  1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a) On or about December 1, 2020 an employee was exposed to the hazard of falling 10 to 21-feet to the ground below and were provided with a Guardian part number 01705A yellow harness, and Guardian yellow lanyard and self retracting life line system. The harness and self retracting life line system contained defects caused by the roller and friction buckles, lanyard buckles and other hardware having excessive corrosion and deformation.
