An employee wore a N95 filtering facepiece respirator without a written recommendation on the employee's ability to wear a respirator. This condition exposed employees to a respiratory hazard.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) The Department of Veterans Affairs at 2500 Foot Hill Drive, Salt Lake City, UT 84148: On and preceding 12/2/20, the employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace. Employees wore N95 filtering facepiece respirators and/or MaxAir Systems CAPRs without medical evaluations. This condition exposed employees to a respiratory hazard. The Department of Veterans Affairs was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1910.134(e)(1), which was contained in OSHA inspection number 1151500, notice number 1, item number 2 and was affirmed as a final order on 9/22/2016, with respect to a workplace located at 1310 24th Ave South in Nashville, TN.
