29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about December 3, 2020, workers were applying stucco and decorative stone to the residential apartments under construction while working from a three tier, tubular-welded scaffold that did not have a ladder for access or guard rails, exposing the workers to a fall hazard.
