29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about December 2, 2020, located at 2685 Canterbury Rd, Upper Arlington, OH 43221, the employer did not ensure that employees involved in framing activities were protected against a fall from the top of a first floor wall frame thereby exposing employees to a fall hazard greater than 6 feet.  Hammer One Construction was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1431911, citation number 2, item number 1 and was affirmed as a final order on October 31, 2019, with respect to a workplace located at 2750 Welsford Rd, Upper Arlington, OH 43221.  Hammer One Construction was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1104058, citation number 1, item number 1 and was affirmed as a final order on January 11, 2016, with respect to a workplace located at Oxford Loop South, New Albany, OH 43054.  Hammer One Construction was previously cited for a violation of this occupational safety and health standard 1926.502(b)(13), which was contained in OSHA inspection number 889723, citation number 1, item number 2 and was affirmed as a final order on April 4, 2013, with respect to a workplace located at 8110 Cliffrose Ct, Plain City, OH 43064.  Hammer One Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 314592874, citation number 1, item number 2 and was affirmed as a final order on May 18, 2011, with respect to a workplace located at 1144 North High St, Columbus, OH 43201.

29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a. On or about December 2, 2020, located at 2685 Canterbury Rd, Upper Arlington, OH 43221, the employer did not instruct all employees in the recognition and avoidance of unsafe conditions while performing framing work . Framers were installing trusses and other framing pieces on a one story home being renovated exposing employees to a fall hazards greater than 8 feet.  Several inches of snow covered the existing roof of the structure.  Hammer One Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.21(b)(2), which was contained in OSHA inspection number 1431911, citation number 1, item number 2 and was affirmed as a final order on October 31, 2019, with respect to a workplace located at 2750 Welsford Dr, Upper Arlington, OH 43221.
