29 CFR  1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:   At the residential construction site located at 4136 NE Dearborn Lane, Lee's Summit, MO 64064:  Employees were exposed to a electrical shock hazard.  Employees were using extension cords at the worksite to power two Skilsaw brand circular saws. The cords were found to have visible damage to the outer jacketing resulting in the internal wiring being exposed. The employer failed to inspect the cords for any external defects prior to their use at the worksite.
