29 CFR  1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:   At the residential construction site located at 4136 NE Dearborn Lane, Lee's Summit, MO 64064:  Employees were exposed to a electrical shock hazard.  Employees were using extension cords at the worksite to power two Skilsaw brand circular saws. The cords were found to have visible damage to the outer jacketing resulting in the internal wiring being exposed. The employer failed to inspect the cords for any external defects prior to their use at the worksite.

29 CFR  1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  At a worksite located at 4136 NE Dearborn Lane, Lee's Summit, MO 64064: Employees were exposed to fall hazards. Employees were exposed to a fall of approximately 9 feet to the lower level floor as a midrail was not installed on a guardrail system for the staircase landing on the main floor. The employer failed to ensure that all guard rails contained a midrail at the facility.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a worksite located at 4136 NE Dearborn Lane, Lee's Summit, MO 64064: Employees were exposed to fall hazards. The employer failed to protected employees from fall related hazards in that employees were observed working at a height of 11.6 feet without the use of any personal fall protection or fall protective systems.  The (KT Lilly Construction LLC) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926. 501(b)(13), which was contained in OSHA inspection number 1340024, citation number 1, item number 2b and was affirmed as a final order on (March 13, 2019), with respect to a workplace located at 500 Wenonga Pl, Lee's Summit, MO 64064.
