29 CFR  1910.147(f)(3)(ii)(D):Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained:    A)	DTE4-1400 Press:  On or about December 10, 2020 and at times prior to,  the employer exposed employees to potential hazardous energies, in that the employer failed to ensure that the machine was locked out by each authorized employee using a group lockout device, group lockbox, or comparable mechanism when more that employee was exposed during maintenance activity such as die changes and calibration.    B)	Simpac SV2P-800 Ton Press:  On or about December 10, 2020 and at times prior to,  the employer exposed employees to potential hazardous energies, in that the employer failed to ensure that the machine was locked out by each authorized employee using a group lockout device, group lockbox, or comparable mechanism when more that employee was exposed during maintenance activity such as die changes and calibration.
