29 CFR 1917.45(k)(1): A designated person did not visually inspect each crane and derrick on each day of use for defects in functional operating components: A) Midwest Terminals of Toledo International, Inc., worksite located 3518 St. Lawrence Drive, Toledo, Ohio, 43612: On or about December 9, 2020, the employer did not ensure that a Liebherr Mobile Harbor crane, Model LHM 280, S/N 140865 was thoroughly inspected by a designated person prior to use. Employees were exposed to being struck-by cargo that was being unloaded from the vessel due to the failure of functional components such as but not limited to the Anemometer. B) Midwest Terminals of Toledo International, Inc., worksite located 3518 St. Lawrence Drive, Toledo, Ohio, 43612: On or about December 2, 2020, the employer did not ensure that a Liebherr Mobile Harbor crane, Model LHM 280, S/N 141004 was thoroughly inspected by a designated person prior to use. Employees were exposed to being struck-by cargo that was being unloaded from the vessel due to the failure of functional components such as but not limited to the Anemometer.
