29 CFR 1910.212(a)(2):General requirements for machine guards. Guard was not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about 12/01/2020, at Line 1, employee operating RMF Mixer, Model MB-1200, with a guard that was not secured in place, exposing employee to point of operation hazards resulting in an amputation of the tip of the left thumb.
