29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On 1/27/2022, an employee of Louisa Food Products, Inc. was operating the Grim Paddle Mixer at a commercial building located at 1918 Switzer Avenue, St. Louis, MO. The employee who is an authorized employee did not install lockout devices before bypassing a machine guard on the Grim Paddle Mixer. While the employee was clearing dough from the Mixer the slide gate activated, resulting in an amputation on the left middle finger.

29 CFR 1910.15l(c): Where employees may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On 2/4/2022 and times prior to, employees of Louisa Food Products, Inc. operating Raymond 7000 Series Stand up forklifts in the Shipping Department located at 1918 Switzer Avenue, St. Louis, MO. The employer did not provide suitable facilities for the quick drenching or flushing of the eyes and body for employees exposed to injurious corrosive materials. Employees remove, reinstall, charge and check water levels in batteries of powered industrial vehicles.
