29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about 12/09/2020 employees at the Greenfield Village construction project were exposed to eye punctures. Employees were installing sheet metal to a fascia board using a pneumatic nailer without eye protection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 12/09/2020 employees at the Greenfield Village construction project were exposed to approximately 10' falls. Employees were installing sheet metal to a fascia board from an aerial lift. The employees were not provided fall protection.
