On or about September 13, 2021 at 1500 Berlin TPK, Wethersfield, CT job site: Employer did not ensure that employees engaged in the residential contraction activities (roofing) approximately 27 feet above ground level were protected by guardrail systems, safety net system, or personal fall arrest system.

On or about September 13, 2021 at 1500 Berlin TPK, Wethersfield, CT job site: Employer did not ensure each employee on walking/working surfaces was protected from falling through holes 9 foot deep. Holes in the roof were 2 feet by 4 feet and 2 foot by 2 foot in the area. Employer did not protect employee from falling through holes more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a guardrail, cover, or fall arrest system. Unprotected hole opening exposed employees to an elevated fall hazard of approximately 9 feet.
