29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): (a) Lamination department, Hymmen laminator 1A102: On or about December 9, 2020, an employee was fatally injured when struck by a moving board during lamination repair/replacement. Energy control steps were not applied so that sources of hazardous energy were controlled prior to performing servicing activities as required by 29 CFR 1910.147(d)(1) through (d)(6) to protect against machine and equipment hazards.
